GelijkGezond focuses on people with low incomes who are dealing with multiple interconnected issues, including debt, health problems, housing insecurity and unemployment. Traditional support systems often fall short because they address these challenges separately.
At the heart of the approach are the so-called “doorbijters” – dedicated professionals who work across systems and sectors to support people holistically. Rather than operating through isolated service desks, they help individuals navigate healthcare, social support, debt assistance and employment services in an integrated way.
The financing model behind GelijkGezond is equally innovative. Municipalities and health insurers only pay when measurable results are achieved. This shifts the focus from processes and systems to meaningful impact in people’s lives.
Over the coming years, 2,000 residents in Delft, Dordrecht, Tilburg and Zoetermeer will receive support through the programme.
