During an appointment, the healthcare professional shows the patient a dashboard with their current health situation. The dashboard also contains data regarding the other practitioners with other organizations, for example the hospital or the pharmacy. The healthcare professional provides explanations and (treatment) advice.
The healthcare professional can assess the historical course of interventions. This enables them to make better and more effective decisions. They can also decide to deviate from the treatment protocol, for example because of given side effects. In that case, the disadvantages are then greater than the benefits for a patient. For example, the medication is scaled back (de-prescribing). In addition, the patient can be included in specific interventions: for example, polypharmacy discussions or encouraging adherence. And finally, medication monitoring becomes more effective. This prevents inefficient overtreatment, for both the patient and the healthcare professional and health insurer.
This way, the patient gets a complete file with more information. Better insight leads to a better clarification on behalf of the healthcare professional on (the connection between) the treatment, outcomes and healthcare costs. This allows the patient to make a more conscious and oriented decision.
A test in the Zuid-Holland North region shows that the knowledge technology infrastructure improves the care of individual patients. It is technically possible to obtain information from the various databases. This provides relevant control information for the patient and for the healthcare provider.
